Two Cases of Delayed Stenting for Symptomatic Carotid Atery Stenosis with Intraluminal Thrombus

Abstract

|
Intraluminal thrombosis in patients with carotid stenosis is rare and its treatment remains controversial. We have experienced two patients with symptomatic severe carotid stenosis combined with intraluminal thrombi. We treated them initially with warfarin, aspirin/clopidogrel, and statin for 6 and 8 weeks. During the period of medical treatment, there was no clinical or radiological recurrence. Follow-up cerebral angiography showed resolution of the intraluminal thrombus. Carotid stenting with angioplasty was performed under distal protection device subsequently. Diffusion weighted magnetic resonance (MR) image obtained the fol-lowing day did not show any new embolic lesions. Our cases indicate another example that delayed carotid stenting and an-gioplasty after a medical treatment including antiplatelet agent, anticoagulant and statin may be a safe and effective treatment.
